A ProActive healthcare professional will come to your home to discuss our services. At that time our healthcare professional will also conduct an initial assessment to gather all of your medical history and assess your medical situation. We will also work with your healthcare professional to create a telehealth program to assist with the management of your chronic disease. Once a telehealth program is established, a ProActive Healthcare staff member will deliver and set up the appropriate products and discuss with you the functionality of each item. Our staff will not leave your home until you are 100% comfortable with your monitor and all your questions have been addressed.
